The company's mission is "to take care and treat patients at home with expertise, ethics and devotion", developing services increasingly closer to the patient's needs.**How will you CONTRIBUTE and GROW?**Fornire ai pazienti a domicilio una suite di servizi (tra cui l'installazione di dispositivi medici e attività di formazione/coaching in base alle loro specifiche necessità), al fine di supportarli nella gestione delle malattie croniche, nel rispetto della prescrizione medica e delle normative locali.L'infermiere case manager specializzato nelle cure domiciliari opera all'interno della Centrale Operativa di Medicasa e rappresenta il punto di riferimento primario per il paziente, la famiglia (caregiver) e l'équipe multidisciplinare. Facilita e ottimizza i rapporti e la comunicazione con i Medici di Medicina Generale (MMG), i Distretti Sanitari e gli operatori del territorio. Monitora la corretta applicazione dei Piani di Assistenza Individuale, verificandone l'appropriatezza clinica, in collaborazione con il Coordinatore Sanitario, e l'allineamento con gli obiettivi assistenziali prefissati. Gestisce l'assegnazione delle attività assistenziali agli operatori, le comunicazioni e le telefonate di natura clinico-sanitaria,



nonché l'interfaccia con gli applicativi di ATS .Assicura il monitoraggio continuo e intraprendente degli outcome clinici e dei dati sanitari attraverso l'utilizzo della Cartella Clinica informatizzata e degli strumenti digitali a disposizione, curando la redazione e la verifica della documentazione e delle relazioni periodiche.
