Ph3Job Summary /h3 pWorks in conjunction with the Director of Behavioral Health and the Clinic Manager to actively and consistently contribute to the clinic operations and communications; fully integrate primary care with mental healthcare; behaves in a manner consistent with the mission and objectives of OCHC and performs duties as requested. /p h3Education Training /h3 ul liMust pass annual competencies /li liActive professional development /li liSatisfactory maintenance of continuing education standards as established by the Division of Professional Registration of Missouri /li /ul h3Licensure Certifications /h3 ul liCurrent licensure of one of the following: ul liLicensed Clinical Social Worker /li liLicensed Master Social Work /li liProvisionally Licensed Professional Counselor /li liLicensed Professional Counselor /li /ul /li liFamily and Friends CPR within 6 months of employment /li /ul h3Experience /h3 pOne-year experience preferred /p h3Job Responsibilities /h3 ul liPerforms screenings/evaluations of individuals for mental health and substance abuse disorders. /li liPerforms brief interventions for individuals with behavioral health problems. /li liProvides behavioral supports to assist individuals in improving health status and managing chronic illnesses as needed. /li liMeets regularly with the primary care team to plan care and discuss cases and exchanges appropriate information with team members in an informal curbside manner as part of the daily routine of the clinic. /li liIntegrates with primary care providers.
/li liSupports primary care physician/teams in identifying and behaviorally intervening with patients who could vantaggi from behavioral interventions. /li liParticipant in front line interventions with first looking to manage behavioral health needs within primary care practice. /li liInterventions /li liIdentifies problem behavior, discusses impact of intervention, and decides what to change. /li liImplements specific and goal directed interventions. /li liConsistently uses monitoring forms. /li liConsistently uses behavioral health ‘prescriptions’ /li liUses multiple interventions simultaneously /li liProvides education to patient and significant others. /li liProvides handouts explaining appropriate information. /li liUtilizes teach back strategies. /li liTailors education to each specific patient and issue. /li liConsistently provides feedback to primary care physician. /li liCommunicates in a brief, concise, and brief manner. /li liMaintains focus on referral question. /li liProvides description of action plan. /li liA. Provides plan for follow-up care. /li liTransitions patients to traditional behavioral health counseling as needed. /li liUses correct visit templates when documenting, differentiates visits between BHC and counseling consistently.
/li liDismisses each patient by walking them to the front office communicating with office staff what the patient needs to be scheduled for and if it is a BHC appointment what number that session will be. /li liEnsures Medicaid prior authorizations are obtained prior to 5th patient visit. /li liProvides comprehensive mental health care to patients per OCHC procedure and policy and collaborative practice agreement/protocol, utilizing supplies and procedures appropriate to ages of the patients serviced including child, adolescent, adult, and geriatric patient population for the subsequent duties. /li liObtains and documents all relevant health history. /li liPerforms mental health examination and documents relevant findings. /li liDevelops and implements a plan of care, with the patient and appropriate others, which may include additional testing, education, counseling, health promotion, referral, and follow-up, and documents in the patient record. /li liEducates and counsels individuals and/or their families on health, health promotion, and illness, based on identified mental needs. /li liProvides mental care related consultation to and collaborates with community agencies and organizations. /li liDocumentation is completed and signed within 72 hours after patient has been seen. /li liProvides correct documentation, diagnoses codes and correct template to ensure correct coding/billing occurs. /li liCorrections from coding are completed in a timely manner. /li /ul /p #J-18808-Ljbffr
