Researchers are investigating the role of torquetenovirus TTV viral load as a possible biomarker for response in patients with metastatic clear cell renal cell carcinoma undergoing first-line immunotherapy treatment. The study aims to descriptively explore how TTV levels change during treatment with immune checkpoint inhibitors ICI. This observational study is conducted at the Regina Elena Cancer Institute. Participants include patients starting first-line immunotherapy,
including combinations of ICI and tyrosine kinase inhibitors TKI. Biological samples such as blood and stool will be collected at several timepoints before treatment, before the second therapy cycle, after the fourth cycle coinciding with the first radiological reassessment, and after 12 weeks from cycle four during further radiological assessments. Additional samples may be taken if immune-related toxicities occur or at disease progression. Participants will have personal, clinical, and treatment data collected from medical records. Sample analyses will be performed to measure TTV viral load and assess the microbiome. The study will monitor these outcomes over 24 months. The procedures do not affect patients standard diagnosis or therapy, and participation requires informed consent. Safety and follow-up data, including disease progression and patient status, will also be recorded during the study period.
