Essential Responsibilities:

- Role modeling professionalism
- Develop positive relationships with LIFE residents, families, and staff
- Daily documentation of activities/goals performed
- Intervene verbally with individuals during behavioral incidents
- Transport residents to and from employment and volunteer sites as needed (mileage reimbursed)

Required Skills/Abilities: Excellent verbal communication, interpersonal, organizational, and time-management skills; ability to function well in a high-paced environment; proficient with Microsoft Office.

Preferred: Experience with individuals with intellectual disabilities or a vulnerable population; high school diploma or equivalent, some college or technical coursework preferred.

- Oversee and support front desk operations, including supervision of administrative support staff
- Manage and maintain the organizational on-call schedule
- Schedule,



coordinate, and document monthly Management and Operations Team meetings
- Support Board of Directors meeting preparation (room setup, recording, materials, tech setup)
- Maintain organizational directories, distribution lists, and org charts

Provide hands-on support, encouragement, and training that empowers individuals to develop workplace skills, build confidence, and become more independent.

Essential Responsibilities:

- Provide individualized job coaching and support at employment and volunteer sites
- Teach and reinforce workplace skills, routines, and professional behaviors
- Conduct routine employment site visits and weekly follow-up checks
- Transport individuals to and from employment sites as needed (mileage reimbursed)
- Complete daily documentation and maintain case notes in I-Centrix
- Provide verbal intervention and support during behavioral situations when necessary

Required Skills/Abilities: Excellent communication and interpersonal skills; strong organizational and time-management abilities; ability to work independently and collaboratively; proficiency with Microsoft Office.

Preferred: Experience supporting individuals with intellectual or developmental disabilities, autism, or other vulnerable populations; high school diploma required, related college coursework preferred.

Case Manager in the Community – Plymouth

Pay: $20.50 – $21.00

Schedule: Tempo parziale -2-3 days per week, Monday – Friday, 10 -15 hours weekly, potential for more hours,



earliest start time would be 9am, and the latest end time would be 5pm

Essential Responsibilities:

Primary responsibilities include, but are not limited to:

- Role modeling professionalism
- Role modeling daily skills being taught
- Daily documentation of activities/goals performed, EX: Case notes
- Ability to provide transportation when required
- Attend and participate in group social activities
- Develop positive relationships with LIFE residents, families, and staff
- Assist and maintain the health and safety of the individual
- Assist with activities of daily living as required re: attending and/or planning activities, menu planning, shopping, cooking, money management, etc.
- Intervene verbally with individuals during behavioral incidents
- Complete required daily case notes to record accomplishments/activities/goals performed from your time spent with the individual(s) supported
- Effectively manage LIFE email account
- Proficient with Microsoft Office Suite and other required software.
- Complete required training as assigned
- Have reliable transportation to bring individuals we serve in the community
- Other duties as determined by Supervisor or Director
- Perform additional duties, or substitute for others as required. Additional duties may include but are not limited to supporting the safety of all staff, EX: clearing snow and ice from walkways, vehicles, parking lots, and roads using equipment like shovels and deicing agents as needed

Preferred Education and Experience:

- Experience working with individuals with intellectual disabilities, or a similar vulnerable population preferred but not required (personal or Special Olympics experience valued)
- Strong computer skills
- Experience with positions that interact in the community–extrovert
- High School diploma (or equivalent) with some college or technical coursework ideal
- Patient and kind composure mandatory!!!

Physical Requirements/Work Environment:

- Must be able to maintain the pace of the individuals we support
- Must be able to lift up to 10 pounds
- Must be able to work in a variety of environments: walking, sitting, standing

Additional Requirements:

- Able to pass all required LIFE pre-employment screening requirements:
- Acceptable reference checks from previous employers
- Criminal Offender Record Information (CORI)
- Disabled Persons Protection Commission (DPPC)
- Department of Developmental Services (DDS) Fingerprint-Based Criminal Background Check
- Motor Vehicle Driving History – acceptable to LIFE’s insurance carrier

- Able and willing to complete all in person and on-line trainings assigned by LIFE
