assisted living facility, hospital, etc.) based on individual needs. The Hospice Social Worker facilitates the provision of supportive services to assist to meet the needs of the patient and caregivers, with a holistic approach, keeping with the patient and caregiver’s beliefs and goals of care. Provides consultation and education for patients, caregivers, and the Hospice IDG Team. The Hospice Social Worker coordinates with the Hospice IDG Team, patient, caregivers, providers, Medical Director, referral sources, community sources and the Hospice IDG members to work toward plan of care goals. The Hospice Social Worker documents in the patient’s medical records in a timely manner according to agency requirements. The Hospice Social Worker provides patient care based on provider’s orders, departmental policies and procedures, regulatory guidelines and best practice.
