Position: Full-Time Physical Therapist in Home Health, responsible for evaluating and treating patients within Warren & Claiborne Counties.ResponsibilitiesPerform patient evaluations, develop and implement treatment plans, and document care following policies and physician orders.Provide patient education, counseling, and coordinate with interdisciplinary team members.Maintain accurate documentation, ensure treatment goals are measurable and objective, and meet productivity standards.Communicate patient progress to physicians and team members, and adjust treatment plans as needed.Qualifications & SkillsMaster’s degree in physical therapy from an accredited program.Minimum nine months of experience as a physical therapist; community/home health experience preferred.Current licensure to practice in the state; valid CPR certification.Valid driver’s license with ability to travel to various locations. xbvzest Key Attributes: Strong clinical skills, ability to work independently, excellent communication, and organization skills.
