/ph3Job Summary: /h3pThe Physical Therapist shall screen, evaluate, re-evaluate, discharge plan,



and make revisions in any established plan of care as supervised by the physician.
/ph3Qualifications: /h3ulliBachelor's, Master's or Doctorate degree in physical therapy from an accredited program.
/liliCertification and licensure requirements as established by state and national board associations.
/liliActive state license required.
/liliExcellent organizational and time management skills /liliExcellent written and verbal communication skills /liliPossess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines.
/liliAbility to supervise Physical Therapy Assistants /liliCompletion of all required screenings, certifications, and licensure.
