Role Overview
As a Hospice RN at VitalCaring,



you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.
You’ll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes. Through each visit, you bring not only clinical expertise, but also reassurance, clarity, and compassion.
What You’ll Do
Lead Patient-Centered Hospice Care
- Conduct comprehensive patient and family assessments
- Develop and manage individualized, comfort-focused care plans
- Deliver skilled nursing care with a focus on pain and symptom management

Serve as the Care Coordinator
- Act as case manager, coordinating care across the interdisciplinary team
- Collaborate with physicians, social workers, chaplains, and aides
- Monitor patient condition and adjust care plans to maintain comfort

Support Patients & Families
- Provide education, emotional suppor
