Schedule
Primarily Monday through Friday, with the flexibility to structure your day around your patients rather than a shift clock
Full time: two weekends per month, with a weekday off following each weekend worked
Part time: one weekend per month

What You'll Do
Provide skilled nursing visits to patients in their homes under the direction of the RN case manager
Administer medications, wound care, and other treatments as ordered
Monitor patient condition and report changes to the RN and physician promptly
Educate patients and families on their care plan, medications, and home safety
Document visits thoroughly and in a timely manner
Build real relationships with the patients you see week after week
What We're Looking For
Active LPN license in Oklahoma
Home health experience is a plus, but not required. We welcome LPNs from hospital, facility, or clinic backgrounds who are ready for a change
CPR certification
Comfortable working independently and managing your own schedule A genuine desire to provide one-on-one, relationship-based patient care
