As a Care Consultant at Baptistcare, you won’t be processing people through a system. Instead, you’ll be walking alongside them, helping them stay safely and independently in the homes they love.

THE ROLE

You’ll be the primary point of contact for a caseload of home care customers assessing their needs, planning their services, managing their budgets, and visiting them regularly to make sure their care is actually working for them.

* Conduct intake, initial, and ongoing assessments of aged care-related needs
* Develop and maintain goal-directed care plans within each customer’s home care budget
* Coordinate and facilitate supports and services across the funded program
* Manage package spend and advocate for customers’ needs and preferences
* Conduct regular home visits and ongoing monitoring to support customers to age well in place
* Build strong relationships with customers, families, and community stakeholders
