What You'll Do Lead Patient-Centered Hospice Care Conduct comprehensive patient and family assessments Deliver skilled nursing care with a focus on pain and symptom management Serve as the Care Coordinator Act as case manager, coordinating care across the interdisciplinary team Support Patients & Families Provide education, emotional support,
