The goal is to reduce recidivism, promote recovery, and facilitate successful community integration. This position leads the FACT team’s daily operations, ensuring that necessary expertise and resources are available to the team to support effective delivery of care to the justice involved population. The Team Leader will collaborate effectively with various stakeholders, including criminal justice agencies, mental health providers, and community organizations. /p /brpThe FACT Team Leader will support the team in promoting recovery, consumer involvement, engagement in individual and group work, ensure compliance with governmental and Agency regulations and operating standards, monitor competition of all records and reports, act as liaison with other Divisions/Agencies, provide support and consultation. The role is responsible for providing administrative supervision of FACT Team staff, monitoring and ensuring productivity is in line with budget revenues and expenses of the program they are assigned responsibilities. /p /brpbCore Responsibilities /b /p /brul /brliManage the daily operations of the Forensic ACT team, ensuring adherence to the ACT model and fidelity to program guidelines. /li /brliProvide direct supervision to a multidisciplinary team (excluding the FACT psychiatric provider), including clinicians, nurses, peer specialists, and other staff, fostering a collaborative and supportive team environment. /li /brliEnsure the provision of person-centered, recovery-oriented services that address the unique needs of individuals with SMI and criminal justice involvement, including mental health treatment, substance use services, vocational support, and community integration /li /brliEstablish and maintain effective partnerships with criminal justice entities, housing providers, healthcare organizations, and other community resources to support comprehensive care and successful community reintegration.



/li /brliEstablish and maintain effective partnerships with a variety of community entities and engage in community outreach to increase awareness and understanding of the FACT program to promote submission of appropriate referrals. /li /brliUtilize data-driven approaches to monitor program outcomes, implement continuous quality improvement initiatives, and ensure compliance with state and federal regulations. /li /brliCoordinate ongoing training for team members to enhance competencies in areas such as trauma-informed care, motivational interviewing, and forensic mental health practices. /li /brliCarries an assigned caseload and serves as the primary therapist to that caseload. Role functions with at least 50% of scheduled work time to require client-facing activities. /li /brliConducts Screenings, Clinical Assessments, Diagnostic Impressions and outlines treatment recommendations for program participants /li /brliDevelops Crisis Plans and support follow up to client emergencies with goals and objectives established /li /brliPromotes the involvement of family and/or significant others in the recovery process /li /brliProvides individual psychotherapy, group therapy and crisis intervention /li /brliProvides Crisis/On call Services as scheduled /li /brliPerforms clinical documentation as required by regulatory oversight government agencies; OMH, OASAS and DOH and participating insurance companies in a timely and efficient manner /li /brliPromotes individualized, consumer driven solution focused therapy experience /li /brliProvide Supervisory and Administrative Duties as assigned: /li /brliMonitors staff productivity and works closely with assigned reports to modify practices to maximize productivity /li /brliProvides administrative supervision to appropriate team members /li /brliAssures staff therapist documentation practices are timely and in accordance with regulatory compliance standards /li /brliRecruits, hires and evaluates the performance of FACT team members /li /brliOrients all new staff therapists to agency culture,



values and mission /li /brliAssures FACT team staff follow agency policy and procedure /li /brliParticipates in community meetings and events as appropriate /li /brliPromotes a work environment that encourages open communication and accountability between staff therapists and the administration /li /brliMaintains adequate clinic coverage and manages crisis/on call scheduling /li /brliActively manages all time and attendance and coverage plan needs for assigned program staff /li /brliWorks collaboratively with the administrative team and senior leadership to develop behavioral health division policies and practices that align with the achievement of the Agency’s strategic goals, mission and vision /li /brliProduces reports for Senior Administration as requested. /li /brliOther duties as assigned and as align with role. /li /br /ul /brpbQualifications /b /p /brul /brliMaster's degree or higher in social work, psychology, rehabilitation counseling, or a related field, OR licensure/registration as a RN (RNs are encouraged to be master’s level or nurse practitioner), or physician. /li /brliPreference will be given to individuals with experience on a multi-disciplinary mobile team. /li /brliMinimum of five years of experience in mental health services, with at least two years in a supervisory or leadership role is preferred. /li /brliExperience working with justice-involved individuals and familiarity with the criminal justice system are strongly preferred. /li /brliMust have a valid NYS Driver’s License. /li /br /ul /brpOur work environment provides the opportunity to grow and learn in a fast-paced atmosphere.
