In this role, you’ll help patients navigate their healthcare journey through clinical triage, care coordination, patient education, and ongoing support that improves outcomes and enhances the patient experience. /p pbWhat You’ll Do: /b /p ul liAssess and triage patient needs by phone and in-office visits. /li liProvide clinical guidance, patient education, and care coordination. /li liSupport patients through transitions of care and post-discharge follow-up. /li liCollaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners. /li liManage medication refills, test results, referrals, and prior authorizations. /li liIdentify preventive care needs and support chronic disease management. /li liHelp patients access community resources and navigate barriers to care. /li liServe as a clinical resource and leader within the care team. /li /ul pbWhat We’re Looking For: /b /p ul liEDUCATION: Graduate of an accredited Registered Nurse program. /li liCREDENTIALS/LICENSURE: Valid, unrestricted RN license in the State of Michigan. Valid CPR certification. /li liMINIMUM EXPERIENCE:
