Our Mission: To empower individuals with behavioral health conditions, substance use disorders, and/or developmental disabilities to achieve the lives of their choosing in the community.
Schedule: 8am- 3:30 pm - (must be able to work 1pm- 9pm 2 nights/week and every other weekend)
Summary of Job Description: Provide direct oversight of Member residential living arrangements to ensure safety, cleanliness, and overall environment of recovery. Support and document Member self-administration of medications.
Specific Responsibilities: Roving and inspection of residences and Member ADL support as needed.
Support with facilities and medication management activities, including (but not limited to) the following:
Disaster Drills
Residential Checklist
Medication Room Checklist
Van Checklist
Implement and document Member interventions and progress relative to Member Treatment Plan as it pertains to medication and ADL compliance.
Complete Incident Reports in accordance with Project Transition/ PCS Mental Health's Policy and Process.
Complete progress notes for any interactions with Members and document in the Electronic Health Record (EHR) (DAP format) by the end of your covered shift.
Facilitate/ Co-facilitate all groups as assigned and complete documentation within 24 hours of facilitating.
Support with transporting Members per the directive of the Program.
Support with observation of Member alcohol and drug screenings as assigned and document observations by the end of the covered shift (if applicable).
Support Members with activities of daily living including (but not limited to) the following:
Daily oversight and coaching of daily living activities.
Observing Member self-administration of medication.
Support with the development of socializ
