Description

Are you passionate about supporting individuals on their journey to recovery and independence? Join our dedicated team as a Case Manager I in the Treatment Apartment Program (TAP), an OMH-licensed scattered site housing initiative. This role is essential in providing person-centered case management and support services to individuals with Serious and Persistent Mental Illness (SPMI), including those with co-occurring disorders and housing instability.

You’ll help participants build life skills, access housing and behavioral health services, and foster community and family connections—all while promoting wellness and stability.

Core Responsibilities
- Implement individualized treatment and rehabilitation plans.
- Provide consultation and foster peer support among participants.
- Ensure safety and upkeep of residences, equipment, and supplies.
- Assist individuals in securing and maintaining stable housing.




- Conduct assessments and develop service plans.
- Coordinate tenancy support, including conflict resolution and agevolazioni navigation.
- Maintain accurate documentation and comply with agency and regulatory standards.
- Provide crisis support in various community settings.
- Work closely with treatment teams, families, and external partners.
- Build and maintain relationships with landlords and community providers.
- Support internal and external reporting requirements.

Qualifications
- Bachelor’s degree in a human-services field, OR
- Associate’s degree + 3 years of experience, OR
- High school diploma + 5 years of experience in human services.
- Minimum 2 years in case management or housing support for individuals with mental health/substance use needs preferred.
- Prior experience in OMH Housing Programs strongly preferred.
- Valid NYS Driver’s License and reliable transportation.
