The South Riverdale Community Health Centre is seeking a Primary Care Asthma Program (PCAP) Educator for a full-time, permanent role in Canada. The educator will work with primary care providers to assess, diagnose, and educate clients and families about asthma and COPD, promoting collaborative management in the community.
The PCAP Educator will enhance awareness, skills, and knowledge of asthma care among diverse communities, contributing to program delivery across multiple CHCs in the area.
