Join our dedicated team as a Case Manager I in the Treatment Apartment Program (TAP), an OMH-licensed scattered site housing initiative. This role is essential in providing person-centered case management and support services to individuals with Serious and Persistent Mental Illness (SPMI), including those with co-occurring disorders and housing instability. /p pYou’ll help participants build life skills, access housing and behavioral health services, and foster community and family connections—all while promoting wellness and stability. /p pbCore Responsibilities /b /p ul liImplement individualized treatment and rehabilitation plans. /li liProvide consultation and foster peer support among participants. /li liEnsure safety and upkeep of residences, equipment, and supplies. /li liAssist individuals in securing and maintaining stable housing. /li liConduct assessments and develop service plans. /li liCoordinate tenancy support, including conflict resolution and agevolazioni navigation. /li liMaintain accurate documentation and comply with agency and regulatory standards. /li liProvide crisis support in various community settings. /li liWork closely with treatment teams, families, and external partners. /li liBuild and maintain relationships with landlords and community providers. /li liSupport internal and external reporting requirements. /li /ul pbQualifications /b /p ul liBachelor’s degree in a human-services field, OR /li liAssociate’s degree + 3 years of experience,



OR /li liHigh school diploma + 5 years of experience in human services. /li liMinimum 2 years in case management or housing support for individuals with mental health/substance use needs preferred. /li liPrior experience in OMH Housing Programs strongly preferred. /li liValid NYS Driver’s License and reliable transportation. /li /ul pThis position typically follows a Monday–Friday, 8:00 AM to 4:30 PM schedule, with flexibility as needed to meet program and participant needs.
