Join our dedicated team as a Case Manager I in the Treatment Apartment Program (TAP), an OMH-licensed scattered site housing initiative. This role is essential in providing person-centered case management and support services to individuals with Serious and Persistent Mental Illness (SPMI), including those with co-occurring disorders and housing instability.

You'll help participants build life skills, access housing and behavioral health services, and foster community and family connections—all while promoting wellness and stability. Provide consultation and foster peer support among participants. Ensure safety and upkeep of residences, equipment, and supplies.

Coordinate tenancy support, including conflict resolution and vantaggi navigation. Provide crisis support in various community settings. Work closely with treatment teams, families, and external partners.

Build and maintain relationships with landlords and community providers. Bachelor's degree in a human-services field, OR Associate's degree + 3 years of experience, OR High school diploma + 5 years of experience in human services.



Minimum 2 years in case management or housing support for individuals with mental health/substance use needs preferred.

Prior experience in OMH Housing Programs strongly preferred. Valid NYS Driver's License and reliable transportation. This position typically follows a Monday–Friday, 8:00 AM to 4:30 PM schedule, with flexibility as needed to meet program and participant needs.
